Section 21: Requests for Communications at Alternative Locations or by Alternative Means
Patients may request that the Callier Center communicate with them in a certain way (i.e., by mail only) or only at certain locations (i.e., at work only). A patient seeking alternative communication accommodations must provide a completed Request Form for Communications Accommodation to the Privacy Officer.
The Privacy Officer will review the request and notify the Patient if the request can be honored. A copy of any request that has been granted shall be included in the Patients Official Medical Record and the billing system. The Privacy Officer will notify any Workforce member or department as required to ensure that a request that has been granted will be complied with.
If the Patient wishes to subsequently revoke the request, the Patient must notify the Privacy Officer of the revocation in writing. Upon receipt of the request, the Privacy Officer will place a copy of the request on the Patient’s Official Medical Record and the billing system, as well as notify any Workforce member or department as required to ensure that a revocation is honored.
